Registered Nurses
Registered Nurses provide leadership on your unit/neighbourhood, consult with your physician as required, and work with other team members to make sure your care plan is followed.
In partnership with you and your family, the registered nurse will develop an individualized care plan to your needs. The registered nurse conducts ongoing assessments, administers medications, and act as a contact for your family members.
Licensed Practical Nurses
Licensed Practical Nurses provide personal health services such as administering medications, performing treatments, changing dressings and other duties to support the registered nurse.
Health Care Aides
Health Care Aides will assist you with bathing, personal grooming, dressing, mobility, and lifts, using the washroom, and mealtime assistance.
Registered Dietitian
Our Registered Dietitian assesses your nutritional needs and works with you to provide menus and meal plans to meet your therapeutic and clinical needs. The registered dietitian assesses and arranges for texture modified diets when needed and ensures your meals are of high quality and are nutritionally dense. We encourage you to discuss your food preferences and cultural needs with the registered dietitian. You and your family are welcome to contact the Dietitian 30 for questions and concerns, or to provide input about your nutritional care.
Occupational Therapist
Occupational therapists encourage maintenance and in some cases rehabilitation through the performance of activities required in daily life. The goal of the occupational therapist is to promote your ability to function to your fullest potential and enhance quality of life. Our occupational therapist will assess you upon admission to determine if you will benefit from any occupational therapy services. Occupational therapists will also assist you by recommending or coordinating adaptive equipment, mobility equipment and seating such as wheelchairs and walkers, lower leg assessments, compression garments, cognitive assessments, and other therapeutic interventions.
Recreation Therapist
The goal of recreation is to help you maintain independence and stay as active as possible. The recreation therapist works with you to identify recreation programs that allow opportunities for self- expression and enjoyment. The recreation therapist plans many therapeutic activities, entertainment and community outings which supports your social, spiritual, physical, and emotional well-being.
Registered Social Worker
Our Registered Social Worker can support you as you adjust to your new home and can assist you with providing information or resources regarding finances and legal decision making such as personal directives, trusteeship, power of attorney. The social worker can also provide counselling for grief and loss.
Pastoral Care Coordinator
In keeping with the mission of our founding Sisters of Providence, pastoral care coordinators work in collaboration with you and your care team to provide ministry, care, and support to your and your loved ones. Whether you have a faith tradition or are more spiritual than religious, our pastoral care coordinators provide support for residents, families, and staff to nurture your well-being. Pastoral care coordinators are available for one-to-one conversations and alongside our pastoral care volunteers also host a variety of pastoral care programs. Pastoral care coordinators can assist you with Advanced Care Planning and choosing a goals of care designation.
Recreation Aides & Rehabilitation Assistants
Recreation Aides and Rehabilitation Assistants work alongside the occupational therapist and the recreation therapist to provide services.
Volunteer Coordinator
The volunteer coordinator recruits and supports a dedicated team of volunteers who provide services to support residents, families, and staff. Our volunteers enhance your quality of life in many ways such as assisting or hosting recreation activities, assisting with special events and community outings, chapel porters, gift shop attendants and adult day program support. Volunteers are not permitted to assist with personal care.
Physicians
All residents of Father Lacombe Care Society are required to be under the care of a physician. Upon your move in the resident care manager or registered nurse will help you select a physician that has “practice privileges” with Father Lacombe Care Society. Physicians 32 are part of your care team and are committed to working with you to support your medical needs and quality of life.
Pharmacists
Medications are ordered by your physician and provided by an Alberta Health Services contracted pharmacy provider. A clinical pharmacist is on-site weekly and works closely with physicians, professional nurses and other members of the interdisciplinary team. The pharmacist may attend your care conference and is available to answer any questions you or your family may have about your medications. For safety reasons, all medications are reviewed every three months to ensure optimize effectiveness, review side effects, and improve your comfort level.
Please discuss the use of alternate therapies, such as herbal remedies with your physician as these remedies may negatively interact with your prescribed medications. For safety reasons, a physician order is required for all alternate or herbal remedies and over-the-counter medications. For legal reasons, professional nurses are not allowed to administer alternate therapy or herbal medications that are not approved by Health Canada and having either a Drug Information Number (DIN), Natural Product Number (NPN) or Homeopathic Medicine Number (DIN-HM). For safety reasons, professional nurses will not administer over-the-counter medications and other medications that are not provided by our pharmacy provider.
